Included below are the registration form and waiver of liability for the Westerly Yacht Club Junior Sailing Program. To get a copy of one of these you need to do the following.

Click on one of the (small) images and it will bring up a larger version of the document. The registration form is on first one, the waiver of liability is on the second (can't you see that :-).

After you click on one of the small images to bring up the bigger version you can either use your browser to print directly to your printer or save it to a file and then print it. It's that easy!
